Sue founded Age Wise Family Services 35 years ago, long before the field of Care Management became what it is today. She has years of experience as a consultant, therapist, and trainer in the field of Aging and Long Term Care. Sue has spent the majority of her career specializing in addressing issues of aging, grief, and loss. Given her extensive experience, Sue has an in-depth knowledge of the history of many of the care communities in the area.

 

She has a unique ability to evaluate an individual's situation and make the best recommendations for that person's care - whether it be provided in-home or in one of the many senior care communities we have in our area.

Sue is in the unique position of advising both providers and consumers of long-term care services.

 

Sue serves on the board of directors of Federation Housing, Inc. She is also a member of the Aging Life Care Association.

Tracey A Miles, MS, CMC

Life Care Manager

Tracey has over 12 years of experience in the area of Care Management, Life Management, and Daily Money Management. She is responsible for providing care management services including developing comprehensive assessment plans, communicating with medical professionals, adhering to medical recommendations for clients, documenting and tracking medication modifications, scheduling medical appointments, arranging for transportation to and from appointments, arranging for in-home care, and providing information regarding community resources available to meet other needs.

 

Tracey has extensive experience in managing medical insurance issues and handling long term care insurance issues. She assists our clients who have difficulty in managing their personal monetary and business affairs which includes bill paying assistance, organizing and keeping track of financial details, banking, deposits, account reconciliation, assist in check writing and on-line maintenance of bank accounts.

Tom Miles, MS, BSW

Life Care Manager

Tom holds a Bachelor’s Degree in Social Work and a Master’s Degree in Criminal Justice. Tom spent 32 years in law enforcement. Retiring in 2017, Tom now uses his extensive social work and counseling background and his knowledge of people to refine and improve the services we provide. In his 32 years in the field, he has focused his energies and talents on assisting individuals and families in problem solving, crisis intervention, and life management.

Tom provides our clients with a unique experience in that he is a male in a female dominated field. While our older female clients love the assistance of a male, our older male clients love a male perspective whenever issues arise. When one of our male clients just want to talk about what they are dealing with, they appreciate the opportunity for male bonding!

Marci Levinson, BA

Life Care Manager

Marci has had a great deal of experience assisting people across the spectrum of living arrangements. She has worked in Nursing Facilities, Assisted Living Communities and provided support for people still living at home. Her focus is on helping people maintain their highest level of independence. Over more than 20 years of working in Nursing Facilities and with Care Management, Marci has worked with clients of all ages with an emphasis on the geriatric population. In addition to focusing on our clients, Marci has experience coordinating with families both near and far, to ensure that clients are properly advocated for and to assure the best possible outcomes for her clients. Marci can assist people with managing disabilities and health issues, while assessing for necessary services. Some ways that Marci can assist clients is by accompanying them to appointments, applying for benefits, moving services and setting up services to support a client.
